A Study of Postmenopausal Women with Intra-endometrial Fluid Collection

Abstract
Objectives: The aim of the study was to assess the clinical significance of intra-endometrial fluid collection in postmenopausal women without any symptoms.

Methods: A group of 26 postmenopausal women with an intra-endometrial fluid collection confirmed sonographically underwent D&C with hysteroscopy. The patients were divided into two groups according to the endometrial thickness (ET). Twenty-two women had thin endometrium (£¼4 mm) and 4 women had thick endometrium (¡Ã4 mm).

Results: When ET was ¡Ã4 mm, proliferative endometrium was found in 2 patients (50%), atrophic endometrium was in 1 patient (25%) and endometritis was in 1 patient (25%). When ET was £¼4 mm, atrophic endometrium was found in 17 patients (77.3%), 4 patients (18.2%) had insufficient endometrial tissue due to atrophic endometrium and endometritis was found in 1 patient (4.5%). A majority of patients with ET of less than 4 mm in ultrasound had atrophic endometrium. The incidence of intrauterine pathology increased with the increasing thickness of endometrium as observed by ultrasound.

Conclusion: The presence of intra-endometrial fluid collection in postmenopausal patients without any symptoms seems to be a benign condition. Normal endometrium of less than 4 mm observed by ultrasound in postmenopausal women without vaginal bleeding does not necessarily need further surgical investigation.
